Community Voices Independent Monitoring for Quality (IM4Q) was
established in 1999 and can be found in all PA counties. IM4Q is
funded by the Office of Developmental Programs through the counties.
Who participates?
Annually 235 persons with Developmental Disabilities, persons who
know them well and their families complete the survey process with
Community Voices IM4Q.
What does IM4Q do?
IM4Q provides opportunities for individuals and their families to
voice their perceptions about services. Their voices are incorporated
in quality improvement plans at all levels; Individual, Provider,
County, State and National level.
Where are IM4Q participants from?
Venango, Cameron/Elk, McKean, and Clarion County and Polk State
Center.
Consumer/Family Satisfaction Team (C/FST)
Consumer
Satisfaction Teams were initially established in 1990, and can be
found in every PA County. Community Voices C/FST conducts confidential,
voluntary, face to face interviews with consumer/family member.
C/FST information is shared with Value Behavioral Health Choices,
Venango County Mental Health Administration, Alcohol / Substance
Abuse Administration and local providers.
Who participates?
This year Community Voices C/FST completed 150 surveys with 14 provider
of Mental Health and/or Drug/Substance Abuse services and supports.
What does C/FST do?
C/FST provides persons and families in Venango County the opportunity
to voice their perceptions about the quality of their alcohol/substance
and mental health supports and services.
Where are C/FST participants from?
Through our survey process the voices of consumers and families
are included in the provision of quality services.
